Under PSIRF you do not investigate every incident in the same way. For each patient safety incident you decide what response will help you learn and make care safer, and you record why. This log is where you record that decision.

It helps you in three ways. Nobody affected is forgotten. Your leadership team, your commissioner and your regulator can see how you made each choice. Over time, it shows you where your risks really are.

• A Word decision record, one per incident, that takes you through the immediate safety actions, any required referral or notification, the learning potential, the response you chose, why, and who decided.
