ECM Questionnaire — Parent & Carer Version

This questionnaire is designed to complement work with the Emotion Communication Map (ECM), and can also be used as a standalone resource.

It is completed by a parent or carer about their child, capturing observations of how their child experiences, expresses, and communicates their emotions — providing a valuable additional perspective to sit alongside the child's own voice in the ECM process.

It is not a diagnostic assessment. It is a structured, neuroaffirming tool that supports parents and carers to feel heard, informed, and part of the process — without blame or judgement.

The questionnaire covers eight sections:

• Internal emotional experience
• Things that bring big feelings and glimmers
• External expression
• How others perceive them
• Understanding others' emotions
• Emotional language
• Tools and approaches tried

When completed, the questionnaire generates a profile which can be printed or saved as a PDF for use in clinic, school, or at home.
