Every teacher has heard this. Nobody hands you the forms.
So you write the phone call on a sticky note. The incident goes in an email to yourself at 9pm, from memory, with the part that happened just before it already gone. Six weeks of reading support exists only in your head, until the day someone asks you to show it.
This is the set of forms for that half of the job — the half that isn't teaching, isn't planned for, and is the half you get asked to account for.
WHAT A RECORD ACTUALLY NEEDS
Official incident-documentation standards ask for eight things: the date and clock time, the location, who was present, what happened immediately before, what actually occurred, what you did, the outcome, and who you notified and when. Most hurried notes carry three or four of them. Page 3 prints all eight as a reference sheet, and every form in the set is built to collect them.

WHAT IS NOT IN IT
No lesson planning. No weekly spreads. No gradebook. No dated academic year. Those exist in abundance already, and none of them are the thing that gets you in trouble.

This is not legal advice, and it does not replace your school's own reporting system — log everything there, exactly as your school requires. What these sheets give you is a clear, dated account in your own hand, written while you still remember it.
Every pupil, parent and colleague in the completed specimens is invented, and the situations are composites. Nothing in this file describes a real child.
FORMATS
Both US Letter and A4, each drawn natively — not one scaled out of the other. Print at 100%, or import into GoodNotes, Notability or any PDF app and write on it with a stylus. There are no tappable checkboxes and no fillable form fields; those don't work reliably in note-taking apps, so the boxes are drawn to be ticked by hand.
HOW TO DOWNLOAD
1. Your files unlock as soon as Etsy confirms payment — usually instant. Etsy occasionally reviews new orders for a few hours and emails you when they're ready. Nothing is lost.
2. Open Etsy in a browser or on a computer, not the phone app — the app cannot download files.
