The aim isn’t to make incident reviews longer. It’s to help make them more meaningful, therapeutic and child-centred — considering what was unfolding around the child, what may have influenced their presentation, how staff responded and what we can genuinely learn going forward.

I’ve found this approach really useful in my own oversight, so rather than keep it to myself, I wanted to make it available completely free to anyone who might find some value in it.
