Print and send family pages in English and Spanish so transition meetings are not a surprise.

I've worked in education for more than 20 years. In that time I've sat across the table from a lot of parents at their child's first transition IEP meeting. Many of them were hearing words like "postsecondary goals" and "age of majority" for the first time, sometimes in their second language. This packet fixes that. Send it home a few weeks before the meeting, and families walk in knowing what usually gets talked about and what they want to say.

Every page has English on the left and Spanish on the right, so you print one copy and the family reads whichever language they're comfortable with. The Spanish uses "usted" and the plain wording that Mexican and Central American families use in the US.

**What's included (16 pages):**

• Cover with lines for the student's name and school year

• How to use this packet

• What transition planning is, in plain words

• Age 14, age 16, age 18, and ages 19 to 21: what usually comes up and what families can do now

• Student voice: All About Me (fill in)

• Family input: strengths, worries, and hopes (fill in)

• Three checklists: work, school after high school, and daily living

• Who to ask about: vocational rehabilitation, the state developmental disabilities agency, SSI, Centers for Independent Living, and college disability services, with lines for local contacts

• 12 questions to ask at the meeting

• My one page home plan: 3 small goals, who helps, and by when

• Words you may hear: a 12 term glossary

**Great for:** transition coordinators, case managers, and IEP teams with Spanish speaking families. Use it before a student's first transition IEP, at parent nights, or as a yearly check in.

**Please note:** This packet explains the process in plain words. It is not legal advice, and it doesn't replace your district's procedural safeguards or your state's rules. Each page tells families to ask their school team about their state.

**Pairs well with:**

• FREE Transition Planning Family Sampler (English & Spanish): 2 pages from this packet.
