The Catheter Care Passport provides a comprehensive, portable record of the person’s catheter, including the type, size, insertion date, and clinical rationale. It outlines routine care requirements, hygiene expectations, and monitoring arrangements. The passport includes clear guidance on red‑flag symptoms, escalation pathways, and emergency contacts. It supports continuity of care across settings, ensuring professionals have immediate access to accurate information. The passport is co‑produced with the person and updated at every review or catheter change.
The Catheter Care Plan outlines the person’s catheter type, the clinical reason for its use, and the level of support required. It describes daily care tasks, including hygiene, bag positioning, tubing checks, and infection‑prevention measures. The plan details hydration requirements, monitoring arrangements, and early signs of deterioration. Clear escalation pathways are included, with instructions for staff on when to contact the GP, district nurse, or emergency services. The plan emphasises dignity, privacy, and person‑centred practice, ensuring the person is involved in decisions about their care.
