SOAP DAP BIRP Session Note Templates - Fillable Pdf + Word | Us Letter A4 A5 iPad
Three ways to document the same session — SOAP, DAP and BIRP — each with a worked example showing how a completed note actually reads, followed by a blank fillable version. Fillable PDF and editable Word, in four page sizes.
Most note templates hand you an empty grid and leave you guessing at the level of detail. This one shows you a finished note first.
▸ WHAT YOU GET
9 pages. 75 clickable fields. Four page sizes, two file formats:
• Fillable PDF — US Letter, A4, A5, and a 3:4 tablet size for GoodNotes
• Editable Word .docx — the same four sizes
• Instant download
▸ WHAT IS INSIDE
1. Comparing the three formats — a side-by-side table of sections, when each one works best, and its main strength, so you can choose deliberately rather than by habit.
2. SOAP — Subjective · Objective · Assessment · Plan
When to use it, how it differs from the other two, a full worked example, then a blank fillable form on its own page.
3. DAP — Data · Assessment · Plan
The fastest format to maintain. Same structure: when to use it, worked example, blank fillable form.
4. BIRP — Behavior · Intervention · Response · Plan
For goal-directed work and funded programs. Same structure: when to use it, worked example, blank fillable form.
5. Appendix — principles of recording: separating observation from inference, marking quotation, data minimization, client codes, retention, and the rhythm of writing notes.
▸ THE WORKED EXAMPLES
Each format comes with a complete, neutral, fictional note — a client presenting with occupational stress, one with family-of-origin material, one working on social avoidance. They exist to show register and level of detail, nothing more. If you have ever finished a session and stared at an empty template, this is the part you actually wanted.
▸ WHAT MAKES THIS SET DIFFERENT
• Genuinely fillable. Real PDF form fields, not a flattened image with lines drawn on it. Tab moves you through the note in reading order.
• Each blank note fits on a single page. No signature line stranded on a second sheet.
• Plain, defensible language, written so the record stays intelligible if a third party ever reads it.
• Word versions are fully editable — change the wording, the colors, the footer, and add your logo to the header.
• Print-friendly. White pages with sage and olive accents, not full-bleed color.
▸ FOUR PAGE SIZES
US Letter and A4 for printing. A5 for a compact clinical notebook — it also prints on Half Letter with your printer's scale-to-fit. The 3:4 tablet size is proportioned for writing with a stylus in GoodNotes, Notability, or any PDF annotation app.
▸ PLEASE READ BEFORE BUYING
This is an organizational tool that supports the structuring of written records. It is not clinical guidance, a standard of care, or medical advice. What a record must contain is set by your state law, your licensing board, your professional association, any payer contract you hold, and your employer's policy. Confirm that a template meets the requirements that apply where you practice before you adopt it. The example notes are fictional and are not case material.
▸ TERMS OF USE
Licensed for use within a single practice, including clinicians you employ or contract. Edit freely and use with your own clients. Not for resale, redistribution, sharing, or publication as a template product.
▸ NEED THE WHOLE SET?
These three note formats are also part of the Private Practice Paperwork Kit — 43 forms covering intake, consent, assessment, treatment planning, records, and billing, including a superbill and a Good Faith Estimate. If you are setting up a practice rather than filling one gap, start there instead.
