About Me

Hi — I'm Lukas, and I should start with what I'm not.

I'm not a therapist. I don't have clients, I've never written a progress note at the end of a long day, and I'm not going to pretend otherwise. What I do is design documents: taking something that has to be filled in, signed, filed, and occasionally read by a stranger — and making it clear.

That turned out to be a useful thing to bring to private practice paperwork. Most forms clinicians work with have been photocopied, forwarded, and half-edited so many times that nobody remembers where they came from. They ask the same question twice, leave out the one thing you needed, and arrive in six different fonts. None of that is a clinical problem. It's a document problem, and document problems have solutions.

So everything in this shop is built the same way: plain language, consistent structure, real form fields you can type into, and Word versions you can rewrite completely. Where a document carries legal weight, I say so plainly and tell you to have it reviewed by someone licensed where you practice.

I'm based in Poland — which is exactly why that notice sits on the sales page instead of in a footer. I'm not going to guess at your state's retention rules, and I'd rather lose a sale than imply I know them.

If something in a form doesn't work the way a clinician would actually use it, tell me. I read every email, I reply within a day, and when someone's right I update the files for everyone who has bought them.
