A 36-resource outcomes, direct-work, independence and transition system for Ofsted-regulated supported accommodation for looked-after children and care leavers aged 16–17 in England. It helps providers evidence young people’s wishes, practical skill development, progress, stability and move-on readiness.

The resources are editable professional working documents. Use strengths-based, age-appropriate language and adapt each tool to the individual young person. Progress may include setbacks; evidence should reflect the young person’s wishes, experience, opportunities and meaningful outcomes. Purchasers must also adapt the resources to their Statement of Purpose, policies, local safeguarding arrangements and commissioning requirements.
