Elopement Plan for Students Who Run or Leave the Area: Team Planning Form, High School
Every team with a runner has the same bad moment: the kid is out the door and three adults are making up a plan while they jog down the hallway. Plan it on a calm day instead, with names, not just roles.
What's included:
• The pattern: where, when, toward or away from something, and the biggest safety risks
• Prevention by area: environment, schedule, supervision, and a safer way to leave
• If the student leaves: who keeps eyes on them, who calls it in, who covers the class
• Space for your school's out of sight procedure
• A confidential staff summary card
• A family page
How I'd use it:
Fill it in with your administrator and the family, and match every step to your school's procedures. This worksheet does not replace your district's emergency plan or required training, and it says so up front. The "safer way to leave" row is where most of the real change happens. Editable Word file included.
The staff summary card is designed to fit on an index card for the people who need it, kept in a confidential spot. The family page helps you plan with home, including what happens there and what has worked. Review the plan whenever the pattern changes or a new adult joins the team.
Who it's for: Teachers, case managers, administrators, and families. Grades K 12.
What you download: one printable PDF, 4 pages, US Letter, plus an editable Word file so you can type in it.
This is an educational resource. It does not replace your school's crisis plan, safety procedures, or any training your district requires.
