Transition Quick Screen: 12-Item Student and Family Intake, Ages 14 to 21
This free transition quick screen has a one page screen that a student and a family member fill out side by side, plus a teacher page that shows you what to use next and a short terms page.
Why I made this: Every fall I sit down with a new student and a stack of forms, and I can watch their focus fade by page two. I wanted one short page for the first day that tells me where to start and gets the family talking before the IEP meeting, not during it.
What’s included:
• A 3 page PDF and an editable Word copy
• The quick screen: 12 short items in 3 areas (learning after high school, work, and living on my own), with a student column and a family column on the same page
• Large checkboxes with word ratings (Yes, Some, Not yet), not faces
• A teacher scoring table for the 3 areas, with a spot to mark where the student and family answers differ
• A "what to use next" map that points each area to the full assessment forms
• A "why I made this" note and terms of use
How to use it: Give the screen on the first day or at an intake meeting. Read the items aloud as needed. The student and a family member answer the same 12 items in separate columns. On the teacher page, count the "Some" and "Not yet" answers for each area and circle any item where the two answers differ. Start your full assessment with the area that needs the closest look.
Who it’s for: High school special education case managers, transition coordinators, 18 to 21 program teachers, and school counselors who want a fast, family friendly first step before a full transition assessment.
Format: PDF, US Letter, 3 pages, plus an editable Word copy. Print in color or black and white.
How this is different: The Transition Assessment + Family Guide Bundle is the full high school set. This free screen is the first page you give a new student and family, so you know which forms to pull next. It does not repeat any of their items.
