Child Interview Form for Social Workers

For social workers working with children: an interview prompt form that assists in asking kids about important areas of their home life.  Helpful for assessing for neglect.  Not intended for use in forensic interviews regarding physical or sexual abuse.

The information on this formed is based on a week-long child interviewing course I took in 2014 taught by Harborview Medical Center's team.  THIS FORM IS MEANT TO BE USED AS A TOOL ONLY.  THE INFORMATION AND FORMAT MAY BE OUTDATED.  IT IS THE RESPONSIBILITY OF THE SOCIAL WORKER TO ENSURE THAT THEIR CHILD INTERVIEW FOLLOWS CURRENT BEST PRACTICES AND AGENCY POLICIES.
