Behavior Accommodations Checklist for IEP and 504 Plans: 96 Supports, High School
When a team says "let's add some accommodations," the same five tend to show up on every plan. This menu puts almost a hundred options on the table so you can pick the few that actually match the student.
What's included:
• 96 behavior and regulation supports sorted into eight groups: environment, instruction, schedule, communication, breaks and sensory, reinforcement, social, and testing
• Check boxes so the team can mark what to try
• A one page summary for teachers so the supports actually get used
How I'd use it:
Bring it to the meeting and let the team, including the family and the student when it makes sense, circle what fits. Then fill in the teacher summary. That page is what gets used on a busy Tuesday. Editable Word file included.
Use the checklist as a menu, not a goal. More supports is not better. Three that every adult actually does will beat twenty that live in a document. Revisit it at the annual review and cross off what you are not using. The teacher summary fits on one page and is meant for gen ed teachers, specials teachers, and subs.
Who it's for: Teachers, case managers, and 504 coordinators. Grades K 12.
What you download: one printable PDF, 4 pages, US Letter, plus an editable Word file so you can type in it.
