Do I really need an index?

The way to decide is to think about what your reader will expect. If your book is a work of history, biography, geography, travel, law, self-help, cookery, finance - anything factual or offering guidance in particular contexts - then your reader is likely to appreciate a good index to help them navigate around the content and pinpoint answers to their questions.
