Progress-Monitoring Binder: Academic and Functional Goals for Secondary, High School
This progress monitoring binder is built for academic and functional IEP goals on secondary and transition caseloads: a data schedule, goal summary sheets, seven probe sheets, two graphs with aim lines, a progress report summary, and decision rules.
Why I made this: Progress reports go a lot faster when the data is already on a graph. I wanted one binder with a probe sheet for each kind of goal we actually write, a schedule so paras know their part, and a simple rule for when to change the teaching. Short probes every week or two tell me more than one long test a quarter.
What's included:
• Cover and how to page
• Data collection schedule
• Student goal summary
• Probe sheets: percent correct, steps done on my own, reading fluency, math accuracy and rate, writing rubric, functional skill checklist, work stamina and pace
• Progress graphs: percent, and count or rate
• Progress report summary with a sample statement
• Decision rules and change log
How I use it: Set up one section per student with a goal summary sheet in front. Pick one probe sheet for each goal based on how it is measured. Write who collects data and when on the data schedule. Graph each probe right away. Look at the graph every 4 to 6 points and use the decision rules page.
How it fits with my other products: This binder is for academic and functional goals only. For behavior goals, use my Behavior Data Graph Templates. For longer job and life skill routines, my Task Analysis Data Sheets have 20 ready made routines. This binder does not repeat either one.
