Summary of Performance Template: Exit SOP for Transition, Ages 14 to 21
This Summary of Performance template gives case managers an organized, plain language exit SOP for students who are graduating or aging out, with student input pages, academic and functional performance summaries, an accommodation history, recommendations, agency contacts, and signature lines.
Why I made this: I have seen SOPs that were a copy and paste of the IEP, and SOPs that were one line long. Neither one helps a college disability office or a vocational rehabilitation counselor. This template keeps the focus on what the next setting needs to know, and it gives the student a real voice in it.
What's included:
• Cover
• Case manager how to page: what an SOP is for, who fills it out, and when
• Part 1: Student information and postsecondary goals
• Part 2: Student input (2 pages, in the student's own words)
• Part 3: Academic performance summary
• Part 4: Functional performance summary
• Part 5: Accommodation history that mattered
• Part 6: Recommendations for education, employment, independent living, community, and self advocacy
• Part 7: Agency and next step contacts, plus signature and date lines
How I use it: Give the student the input pages early in the final year so they have time to think. Fill in the academic and functional pages from current data, keep the language plain, and list only the accommodations that really made a difference. Many states have their own SOP form or required items, so check yours first and use this to gather and organize the information.
How it fits with my other products:
• This is a fill in template for the SOP document itself. It is built to work alongside your state's required items.
• Pairs with the Postsecondary Accommodations Guide in this series, since many disability services offices ask to see the SOP.
