A good behavior plan is one a sub could pick up at 7:45 and follow. Most of the plans I have seen in binders could not pass that test. They were long, vague, and written for a compliance check instead of the adults who actually work with the kid.

What's included:

• A fully completed example plan for a made up 4th grader, so you can see what "specific enough" looks like

• A blank plan in the same order: behavior on video, when it is likely, what it gets the student, prevention, the replacement skill, adult responses, and review

• Prompts in each box that tell you what to write, in plain words

• A section for what adults do when the behavior happens and what they do when it does not

• A review page with the data you will look at and the date you will look at it

How I'd use it:

Use your district's required form when there is one. This works as the draft you bring to that meeting, or as the plan itself where you have room to choose. I would fill in the example section with the team first, out loud, before anyone touches the real student. It shows people how much more specific they need to be. Editable Word file included.

Who it's for: Teachers, case managers, school psychologists, and administrators. Grades K 12.

What you download: one printable PDF, 5 pages, US Letter, plus an editable Word file so you can type in it.
