A team meeting goes better when somebody brings one page of tallies instead of a pile of memories. This log keeps it to one row per incident, and it tracks something most logs skip: whether the adult response was right sized.

What's included:

• Landscape incident log: date, where, what heated it up, how hot it got, what adults did, whether the response was right sized, too much, or too little, and minutes to settle

• Monthly pattern grid by time block and day of the week

• Monthly counts box

• Team review questions and a "one change for next month" line

How I'd use it:

Have whoever was there fill in the row the same day, even if it is short. At the end of the month, tally the grid and look for the boxes that light up. Nine times out of ten the pattern is a time of day, not a kid. The Word file is included for teams that log on a laptop.

Look at the adult response column as hard as the behavior column. That is usually where the pattern hides.

Who it's for: Teachers, paras, and administrators. Any grade level.

What you download: one printable PDF, 2 pages, US Letter, plus an editable Word file so you can type in it.

This is an educational resource. It does not replace your school's crisis plan, safety procedures, or any training your district requires.
