HOW IT WORKS

1. Open the PDF in free Adobe Acrobat Reader, save a copy with your parent's name, and fill in the "In an emergency" page first.
2. Print the emergency page for the fridge and give a copy to each emergency contact.
3. Fill in the care profile and medication list with your parent, then ask the pharmacist or doctor to check the list.
4. Add your family to the tracker and share it (for example in Google Sheets) so everyone sees tasks, appointments and refills.
5. Once a month, go through the Monthly Care Review page together and update both files.
