Job Site Self-Monitoring Checklists and Daily Work Log: Transition and Vocational, Ages 14 to 21
These job site self monitoring checklists and daily work log help students check their own work and behavior at job sites, so the job coach can step back. There is a student versus staff agreement sheet to show you when it is time to fade support.
Why I made this: The goal of every job site placement is for the adult to step back. That only happens when the student can check their own work. These tools move the checking from the job coach to the student, and the agreement sheet gives you data for the IEP.
What’s included:
• A teacher page with setup steps
• A daily work log
• A weekly work checklist: before work, during work, and end of the shift (11 items)
• A picture checklist version for students who need picture support
• An interval self check sheet for "Am I working?" checks with a timer or watch
• A student and staff agreement sheet with a percent formula
• A weekly reflection page
• Four pocket cards
• An example of a filled in work log
How I use it: I pick 3 to 5 checklist items for each student, fill the log out with them for the first few shifts, and then have them rate themselves before I do. When our ratings match most of the time for two weeks, I fade my prompts.
Skills covered:
• Self monitoring at work
• Being on time and ready
• Staying on task and asking for help
Who it is for: Transition teachers, job coaches, and paras supporting students at job sites, work based learning, or school jobs, grades 9 to 12 and 18 to 21 programs.
What you download: Job Site Self Monitoring Checklists Daily Work Log.pdf (12 pages, US Letter, print ready) plus an editable Word version.
How this is different: Task Analysis Data Sheets are staff data sheets. Job Site Task Cards Bundle has site specific checklists. Student Self Monitoring Checklists | On Task Behavior + Job Site Self Check is a shorter on task list. This set is a full student facing system for any site: daily log, 11 item weekly checklist, picture version, interval check, student versus staff agreement, and supervisor form.
