Every barre chord, every long practice session drives the base of the thumb into the neck of the guitar. The carpometacarpal joint, where the thumb meets the hand, is a small saddle joint built for mobility, not sustained compression. When it is loaded repeatedly without recovery, the surrounding tissue locks short and the joint space narrows. Thumb pain is almost always a victim of a locked-short forearm or a weak wrist. You cannot fix the thumb without addressing what is upstream of it.


This protocol works through three phases:


Release: soften the thenar eminence and the deep forearm muscles that are driving the thumb into compression before you even pick up the guitar. 

Reset: create space at the joint directly and restore the neural pathway mobility that underpins precise thumb control.

Rebuild: teach the thumb to hold its position under load without recruiting the forearm, so the strength lives in the right place.
